Purpose
The purpose of this assignment is to provide the student an opportunity to complete the professional activity of obtaining Institutional Review Board (IRB) determination for their DNP Project, which was completed in NR702. The intent of this application and approval is to ensure protection of human subjects and confidential health information. Completion of the DNP Practicum Form and IRB Prescreening Form has several required components and frequently requires revision of the forms prior to final approval. IRB approval of the DNP Project or IRB determination that the project does not constitute human subjects research as defined by the IRB is required prior to implementation of the project.
Course Outcomes
This assignment enables the student to meet the following course outcomes:
CO 2: Demonstrate effective project management strategies (POs 2, 3, 6, 7, 8).
CO 4: Apply theory and evidence from management, policy, and perspectives to support quality improvement and patient safety in healthcare systems (POs 3, 4, 6).

DNP Practicum Form and Related Documents
Please submit the following items:
- Completion of the DNP practicum form (through Section 4: NR705) outlining your project protocol for implementation. NOTE: The DNP Practicum Form was started with your practicum application and project navigation. Completion of the required information is necessary in order to move your project forward.
- IRB prescreening form.
- Letter of support and IRB approval from project site.
- Copy of your NIH or CITI Training certificate showing completion of all required modules, which you.completed in NR701-should be less than 5 years old.
- All data tools for data collection for project evaluation, such as questionnaires, surveys, interview questions.
- Approval for using or adapting pre-established questionnaires or surveys from the tool’s author/owner, unless the tool is available in the public domain, including validity and reliability of the tool (referenced in the DNP practicum form).
- Informed consent or Project information form (if required).
- Recruitment materials (if required).
- Education plan (included in the DNP practicum form).
- Obtains Chamberlain University IRB Prescreening determination.
- All work must reflect doctoral level writing skills.
- APA formatting and style, where applicable.
